THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Simply Aligned Chiropractic is committed to protecting the privacy of your Protected Health Information ("PHI") in compliance with the Health Insurance Portability and Accountability Act of 1996 ("HIPAA").

How We May Use and Disclose Your PHI

We may use or disclose your PHI for the following purposes:

  • Treatment — to provide, coordinate, or manage your chiropractic care and related services, including sharing information with other healthcare providers involved in your care.

  • Payment — to obtain payment for services, including billing your insurance and processing claims.

  • Health Care Operations — for activities such as quality assessment, staff training, and business management.

  • Required by Law — when required by federal, state, or local law.

  • Public Health and Safety — to prevent a serious threat to health or safety.

  • Business Associates — with third-party vendors who help us operate our practice (such as ChiroUp for patient portal and exercise services). These vendors are contractually required to protect your PHI.

Uses That Require Your Written Authorization

Most uses or disclosures of psychotherapy notes, uses for marketing, and any sale of PHI require your written authorization. You may revoke this authorization at any time in writing.

Your Rights Regarding Your PHI

You have the right to:

  • Inspect and obtain a copy of your medical records

  • Request corrections to your medical records

  • Request a list of certain disclosures we have made of your PHI

  • Request restrictions on certain uses or disclosures

  • Request confidential communications (e.g., by mail only, at a specific phone number)

  • Receive a paper copy of this Notice

  • Be notified in the event of a breach of unsecured PHI

Our Responsibilities

We are required to:

  • Maintain the privacy and security of your PHI

  • Provide you with this Notice

  • Follow the terms of the Notice currently in effect

  • Notify you if a breach occurs that may have compromised your information

Changes to This Notice

We reserve the right to change this Notice and to make the new Notice effective for all PHI we maintain. Updated notices will be posted in our office and on our website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
